Welcome to the forum.You can get the Chronic Prostatitis Symptom Index (CPSI) here.
Erectile dysfunction is not typically a part of prostatitis, even though ejeculatory pain is. Did the ED precede the other symptoms? Which reminds me: Which other symptoms do you have? Are there other reasons to explain your ED: Diabetes, depression, spine injury ....... etc. And also: Which treatments have you had for your prostatitis?
Perhaps you can add more detail to your profile. You can edit and add a photo through the Settings button on the upper right.
