Cost of banking sperm/physician's permission?

Any one knows the cost of designated storage of sperm and whether we need a doctor's permission for doing it?

Any IVF clinic will provide this service. The cost varies in different countries.
The last time this came up, for one of my patients from New York, I had to write a prescription before the bank would work with him. He's young, early 30s, and is seriously thinking about surgery, so this is an important step for him.
How long can we keep the sperm frozen without damaging it?
My IVF clinic gives you the option to keep it for 55 years so I presume it can last that long!!
