    Dr... Thank you so much for writing me. Very much appreciated. I am a 58 to male with hx of BPH x 8 yrs. Been on flomax most of that time. I scrub in OR and had to break scrub after 1 1/2 hr cases. Usual symptoms especially at night. Decided to have Greenlight Proc due to low complications  etc. Had proc one month ago. Cath came out after day 1 and immediately started bleeding. After full day I obstructed and went to ER for catheter. Wore it for 4 days and removed it. Was fine after first 7 days but then started pain in perineum ESP with urination. Pain got progressively worse, Dr office treats me with Azio with no help. Pain gets really bad over next week. On third week finally get in touch with doc who says I have low grade prostate infection.. Cipro started for ten days but no help. Pain increases ESP with finishing stream. Last sat  which was day 23 I developed severe pain and passed huge clots with lg amount of blood. On call doc says usual stuff and go for cath if obstructed. Well 2 hrs later I was. Brutalized in ER as I was cath 4x and blood everywhere. Pain was so severe I was nauseated and BP over 200 pulse 110. Went home with cath and obstructed 8 more times over 2 days and irrigated myself. This was horribly painful each time. Saw my urologist finally yesterday and he did office cysto on me and had huge meaty clot attached to raw area where my prostate once was. Alot of raw tissue and he broke clot loose with scope. I was able to painfully pass it. Yesterday and today I have severe pain with starting flow but more when finished esp in peri anal area. I'm so so frustrated as this was an elective procedure. I'm very much afraid my prostatitis will not totally go away and now I have a painful issue for life. Plus I'm concerned over all antibiotics. Fortunately I have only pain and frequency issues not obstructed. Any help Dr would be a help. Seriously it has depressed me deeply. Thanks again. I def have an appreciation for men with prostatitis.     Doc thank you I will. I went to my urologist Monday and he did office cysto on me. Where my prostate was sittin near neck of bladder looked pretty " ratty" and had huge beefy clot there. He knocked it loose and almost killed me passing it.. I am glad I don't have to have a baby. Lol.. Anyway he stated to stay on cipro at least another3 weeks with sitz  baths but I must say pain with and ESP after urination is getting more intense and after flow is over I hurt fir ten more min. I am so frustrated that I had an elective proc done and now headed into my fifth week I am worse. Thank you for checking back with me.

    No sir... Just said it's a long process... Never had anyone have this rough of time.. I will hate him for 2-3 mo but then love him!!! Well he is right with first part.. Lol. 
    When I first started with these symptoms I was ten days post op. Called office and got run around for ten more days till he responded and started cipro again.

    lower abdominal pain; lower back pain; painful urination/ejaculation; golf ball pressure. The main effect of the massages (which I began at home before going to Tucson) was the reduction of the size of the prostate, which it had all kind of benefits: better urinary flow; almost normal erections; lack of of golf ball effect.     No, this all started due to the injection I had at the ED clinic. A terrible decision on my part to go to the clinic! I am 57 years old. Before this happened I was in excellent shape (my main hobby was competitive running). Now I'm taking zoloft and trying not to lose more weight! I think my ED was phycological, so there was no need to do go there. I've never had any surgeries, except a hernia operation approximately 11 years ago.

    Thanks to read me . The surgery was to remove a 'Thornwald' kist. Of course I got a catheter for about 24 hours. .

    Today, I am experimenting essential oils combination in suppository. No promise at all, but who knows? I'll let you know the outcome.

    Hi, Arnon. Have to say that on a scale of 10, pain reduced  down to 3 since the fourth day of the cure with EO's. I just needed two days to adjust the dosage. Initially 130 mg which caused nausea, I cut suppository in two parts, twice a day.

    After a week of treatment I feel so good that I succeeded with a sexual intercourse. My urologist prescribed me a 30 days cure while I am doing a lithotripsy once a week  during five weeks.

    The only side effect is the anal irritation due to suppository. Nothing compared to pain relief I was looking for since more than 5 years. I do relive myself !

    Yes, in the beginning, I was treated with Ciprofloxacin. It diminished the symptoms for just a couple of days,

    , then the relapse came. Just to add some data: PSA is 0.8, urine test and cultive are normal. US of the urinary system shows everything is OK. Testicle US shows a very very small varicocele in left scrotum, US doctor said it is almost insignificant and improbable of causing pain .

    Symptoms keep coming and going. For instance, I haven't had any difficulty while orinating already for 10 days. Nor I got lower back pain or butt pain.  But instead got left testicle pain, specially if pressed somehow, by underwear or while driving. When the butt pain comes back the testicle pain dissapears and so on. The pain seem to move from one area to the other. After sexual relations there is a discomfort/mild pain in scrotum and the side of the leg in front of it, usually the day after. In 3 months, I got only 2 episodes of pain while ejaculating - beyond that, there is mild discomfort in scrotum right after sexual intercourse, and it becomes a little more painful the day after.

    Why is all the pain concentrating in the left side of the pelvic area? (the only exception were a couple of days with pain in both the left and right side of butt. The majority of the time pain is in the left scrotum, left leg, left butt, left side of pennis).

    I have maintained the anti candida diet for almost 8 weeks now and I believe I am almost free of the condition ,I might consider asking my GP for a short course of Fluconasole because I can feel very slight signs of the fungus which is probably just hanging into the prostate  by a thread but if that isn`t completely eliminated it will just start growing again when I start adding stuff back in my diet ,which I must start doing soon because one feels the lack of a bit of sugar etc.

          I would strongly urge anyone who has run out of ideas for treatment to at least try dieting the key element in a diet like this is it requires the very strictest application ,absolutely no cheating ,it`s pretty daunting to start with but one adjusts fairly quickly and I have to say I am thrilled with results and have started to enjoy life once more.

    Yes my history seems related to the weightlifting on the surface, but as always there are a lot of variables.  For example, I had done this same type of lifting in the past with no flareups....I had been doing the same exercises for 6 weeks without incident when my condition slowly came on over the course of a week or so.

     

    On the other hand, both times it seems like lifting seemed to trigger or at least compound the problem.


    Hernia has been ruled out by physical exam and two CT scans (pelvic with oral contrast and appendix scan with contrast administered via enema - was that a horrific procedure!).  PCP doctor referred me to urologist who also confirmed non-infectious prostatitis diagnosis but it didn't seem based on anything but his intuition and experience.  Also went to the ER last week and they found no problems via blood and urine tests and the appendix CT.  ER doctor suggested a gastroenterologist which I have yet to visit.

     

    Can you give me some more info on how you would recommend following up to see if this is a spinal issue and not related to prostate?

    No, I haven't been seen by a rheumatologist yet, but I'm going to visit one in a several days. I'm going to discuss my health problems ( mostly prostatitis now ) with him and do some specific tests specially ones which indicate predisposition to reactive arthritis. I will ask if I can benefit from medication used in rheumatical illnesses. Thank you for advice. Best regards. Pawel

    I appreciate the suggestions, Dr. Arnon.  I ended up in the ER recently because my pain was getting more acute and just felt really "wrong".  Have had probably 4 CT scans in the past month and no issues found anywhere.  Testicular ultrasound normal, all bloodwork and urine tests again come up negative.  On the plus side I had some cardiac tests done which I needed to get done and there were no problems detected with the heart!

     

    So my pain usually has a testicular component, a muscle-feeling component when it has flared up the two times (again both after hitting the gym a couple times doing squats and deadlifts among other things - no maxxing out, just putting up 5 reps).  The pain is not always in the same spot.   I may wake up with my left teste hurting and maybe feeling that pain radiate up into the pelvis somewhere, then after lunch and sitting around all day I may get find that the pain is now exclusively in the area right around the appendix and my testes don't present anything??  Then later it's mainly in my perineum area or it may feel like something is pinching my sphincter.  I don't understand why it's so transitory??

     

    As of yesterday I have developed the biggest muscle knot in my life on the right side of my lower back.  I'm assuming its from being fairly immobile for so long, but maybe a spinal issue could cause all of these symptoms?

    I should add that this 2nd round of symptoms has mostly subsided and is only now minor teste pain which could be on either side at any given time and minor discomfort on the right pevlis - along with the back pain from the huge knot.

     

    Both flareups lasted about 9 days.  I don't know if that's significant..

     

    One last question.  Is there ANY WAY one could have a bacterial infection of the prostate or anywhere else and NOT have any red flags on multiple urine tests and blood tests?  I'm questioning why the doc still has me on Cipro when no evidence of a bacterial infection is present?

    hay Arnon,,, not sure if you remember me, I had pretty severe symptoms about a year and a half ago, went and saw you a couple of times, just giving an update.. I myself believe in my case it was a stress disorter and or muscle-pelvic floor disfunction related, I know every case seems different and Im sure lots of people have infections in the prostate (why else would having it removed, remove major symptoms) Anyway just wanted to say if people are having issues and desperate ( I know how it feels, cant seem to get any answers, oooo god it can be so horrific.) to try going to physical therapy , pelvic floor physical therapy... I developed severe anxiety from the situation, now sure what started first, pain, anxiety, more pain more anxiety, cant get any answers, antibiotics after andibiotics, pain anxiety etc. etc..........SO what helped me was physical therapy, cymbalta which helps with the anxiety and also nerve pain, ( lyrica has helped with nerve pain also) and valium which helps as a muscle relaxer, the combination helped me soooo much. At this point i am proud to say im not taking anything... What else helped was this website, talking to you cause you care about this where all other uros. seemed bothered ( I did learn that I had a big varicos vein, vericosecele, how ever you spell that,) i did have the operation to take care of that, not sure if that helped, i seem to think it had nothing to do with any of my symptoms.) And talking to other people on this site helped a great deal also. So if you want to tell any of the members to contact me, I have reached out to others as well, Im more than happy to tell my side of this story. Would love to help anyone I can.     My husband was diagnosed with Chronic Prostatitis last year after having suffered tremendously from the symptoms since about Nov/Dec 2009. We know so little and it seems like his doctors know even less than that. We also have no insurance and not a whole lot of money to continue to pay for the very expensive uro appointments. The last appointment consisted of the doc throwing his hands, quite literally, in the air... Now they want him on pain management with a different doctor. A neurologist. There are other, new symptoms that just popped up but I'm not sure if they are even related. He is very stubborn, too, by the way. This is very difficult and I'm just trying to understand it better. I thought I should sign up here. I noticed a group for wives and partners... but I see there's not much activity there, now.

So far we haven't even tried getting in to see Doctors in the Tampa area. We are about 40 miles north of there. Lack of knowledge by the local medical community has us feeling pretty frustrated to say the least. We would love to find a magic cure, but at the very least, at this moment, my poor husband just needs some relief. He is completely and totally debilitated.     No, I don't know my score.  I have been dealing with prostatitis for 6 Years now.  Every urol I see doesn't want to do anything about it but put me on antibiotics.  They don't help a bit.  I'm in bad pain with this every other day of my life and It's putting a strain on my marriage and family as well.

    Any suggestions, I will defenately try anything.  I was trying Quercetin but didn't help.

    Thank you for your message.  I am writing out of sheer desperation and fear.  My husband (53 y.o.) has been having some strange symptoms for about 7 weeks.  His symptoms are:  Lower abdominal pain, lower back pain, pain in his penis, rectum, scrotum, testicles and the entire groin area, in general.  He reports that it hurts (the bones in his groin area just to sit down).  He also has some pain upon urination and ejaculation.  His doctor suspected Diverticulitis and put him on a course of antibiotics (Doxycycline and Metronidazole), which he just finished, after 10 days, to no avail.  His urologist did see some blood in his urine, which may be attributed to a kidney stone that they saw on u/s (has history of stones).  He reports, however, that this is unlike the intense pain he has had from stones.  The urologist did a bladder scan and said bladder is emptying fine.  She did a prostate exam, as well.  She said she was "stumped".  His GP ordered a ct scan this week, which showed only a "resolving Colitis".  He has not noticed any blood in his bowel movements, however, and from what I read, that is the most prevalent symptom.  We are both experiencing terrible anxiety and fear.  I am worried that he has advanced bladder cancer because I read that it can cause these symptoms.  I believe an abdominal ct scan (which is what he had) would not look at the bladder, correct?  When reading about the symptoms of Prostatis, I thought it would be worth learning more.  Any thoughts that you may have would be GREATLY appreciated.

    The ct scan he had this past week showed a "resolving Colitis".  He is seeing a Gastroenterologist this coming Wednesday.   I am not a doc (just play one in my house), but, symptoms do not sound related to Colitis to me.  What do you think?  I am not sure about the urine cytology.  His Urologist did examine under microscope (is that what she would have been doing?).  His GP told me today that the urinalysis was ok.  I do not believe he had a Cystoscopy.  Again, the GP told me this morning that bladder on ct was fine.  Maybe we should ask for cystoscopy?

    Yes, I've mentioned possibility of reactive arthritis to the doctor who is in charge of my treatment. This is a general practitioner though. She does not seem too willing to refer me to specialist doctors yet as my blood tests, urine tests, MR of knees, lower part of spine and hip area were all normal. Chlamydia test was also negative after treatment. I have only slight pain in knees - mostly after trying to jump, run. So the doctor assumes that if it was or is reactive arthritis it is dying out and the prostatitis I have will also die out or is not part of the reactive arthritis. Probably I shall put more pressure on my doctor. I didn't question her decisions so far as reactive arthritis according to doctors goes away on its own in most cases. I'm going to have three weeks off in June and already booked consultations with a good rheumatologist and additionally urologist to discuss treatment options.

    Dr. Krongrad, I have some questions

    1. Is cystoscopy essential to check if there are some scars or obstructions in urethra or sth wrong with the bladder? Sometimes the urine stream is quite normal, sometimes it is weak and thin. The more stabbing in anus area and perineal area, the weaker and thinner the stream. Does it mean there is no obstructive factors anatomically and obstruction comes from swelling of prostata when it gets more inflamed?

    2. Is transrectal ultrasonography more precise then the one through abdomen?

    3. Have you had or heard of patients with reactive arthritis or some autoimmune diseases who benefited from methotrexate or other similar drugs used in autoimmune diseases?

    Thank you for the e-mail.  I just saw a 2nd urologist who confirmed that I have chronic prostatitis.  He said that my prostate level was 2.1; gave me a digital rectal exam to make sure there were no other issues with the prostate.  I mentioned to him that my biggest concern is the burning after urination and ejaculation.  He did not have any suggestions as to what course of action, except to stop taking all medications, except ibuprofen (which I have been taking for a low back problem for several years).  Any suggestions would be extremely helpful.  Being a truck driver, the sitting is driving me insane.  I am going to take a different course of treatment; by taking natural products; anti-inflammatory formula, probiotic, quercitin, and prostate formula.  Oh...by the way, the urologist told me to do "stretches" but didn't show me what ones I should do.  I mentioned to him that I work for a large freight company and do a lot of physical work...what type of stretches did he recommend.....a

    I have tried mostly antibotics some steroids and alfa blockers and a drug called avadart. I serioulsy have had so many test and most say I have chronic prostitis and I do think thats what I have. One think makes me think I may have something else. Sence day one two years ago my bowl movement have change and today my main complant is diggestion as my stomach always hurts and I dont go regular at all. I have seen a gi twice and they cant find anything. It seems to go hand in hand with my prostitis symtoms as in poor flow, sore sitting and grown , and when I ejaculate i am either very slow flow or nothing at all. I kinda have been jumping around from doctor to doctor trying to get results and I think I am about to try and pick a doctor regardless of results. Any suggestions I would be greatful and anyone with bowl problems I woulod love to hear. Seems like when my colonoskopy was clear they just dont want to looking in to anything else. Seems to me there could be other things going on that you may show in that test. thanks for your time

    Ya i was aware yet I want to get better. I was on all kinds of meds for the first 15 montuhs and sence have not been on anything other the alfa blockers and avadart which is hard to tell if they even work because of the wax and wains of my symptoms. It is actually flusterating as I havnt been able to find a doctor who would tell what form of prostitis I have and sence its been two year they say chronic. I would say I am not as bad as some I read ablout although my quilty of life went from a 10 to maybe a 2. I am in michigan and I wish there was someone here that  could even help me with pain managment and a plain to help get a little confort. Who know maybe a couple of years down the line I will be ready for surgery.

    what would you suggest in a case like mine? I am 37, married 3 years ago, no children and I would like to have at least 1. What would you suggest to try for someone like me? LRP doesn't sound to hard/risky for someone like me?

     

    I've tried all conventional treatments, but nothing works. That's why I'm desperate, because really this desease makes your life like hell, you can't work well, you can't enjoy the normal things that life offers, it is a non-stop battle against pain and desperation.

     

    I'm for trying injections as I've heard from my doctor that some times people get cured.

    no i have not had a culture as yet, i see my urolagist on the 26.8,2011 should i ask to have this done? i have never had non specific urethritis or urethral discharge, as for trauma  i did have a double groin hernia repair about 15 years ago but nothing recent.

    Hi Dr Krongrad,

     

    Thank you for this site and the information about prostatitis. 

     

    You kindly asked me if I had PSA checked and whether I had trauma, history of cancer or vasectomy. I have had PSA checked many times - always it being in normal values. I have not had a vasectomy and I don't know of any history of cancer in my family.

     

    When I got prostatitis 20 years ago for the first time, it began with blood in semen. My symptoms have always been gnawing pain in the perineum area and the feeling like a lump in the rectum. My first bout took about a year to get rid of with many different antibiotics used. I was then living in England and was being treated by a research urologist at Oxford University (attached to the John Radcliffe Infirmary).

     

    I've lived in Calgary Canada for 18 years and my urologist gives me flomax (useless) and he just shrugs saying that there isn't a cure for this problem. He referred me to a physio who does pelvic floor massage and told me to take quercertin. I have not had the PF massage yet.

     

    My symptoms generally go away after a few weeks of levaquin and diclonefec. The problem for me is that bouts of pain etc are always caused by ejaculation. This of course means I rarely engage in this practise. I get temped once in a while when my wife says "maybe it's okay now" but truth is, whatever is broken seems to stay broken.

     

    When I do ejaculate, the pumping action behind the mechanism (maybe it has a name) is very weak and so it sort of feels like there is a constriction. The liquid does not exit like it used to when I was younger - it just flows out gently.

     

    Here is another strange behaviour. The last time I went to the ER for a very acute bout (two years ago), the ER doc gave me percocet (cetaminophen and oxycodone). Taking these and anything with codeine (tylenol 3) makes the pain increase. My urologist is stumped - I can tell he thinks I have lost the plot. I had sinus surgery two years ago and had no prostate issues at the time. After surgery, I was given percocet and the prostate pain started to present itself about one day later. I stopped taking them - the pain subsided.

     

    I have no trouble with urinating. I don't have to urinate frequently at night and have no pain in the tip of the penis. Usually at the beginning of a bout (like I have this week), I get chills and sweats - kind of like flu for a two or three days.

     

    You asked about trauma. When I was 18 I had a motorcycle accident. The result was some internal injuries and so I had my spleen removed and my left kidney is smaller as it was torn and was sewn back together. Today it functions perfectly as far as they can tell. I have a mid-line incision from just below left lower rib to one inch below the naval.

    Hi Dr. Krongrad.  I tolerated Cipro fine during the first bout, but about 1/3 of the way through the second episode about 1 year later, I had extreme discomfort and what I would call CNS discomfort.  Extreme anxiety and some involuntary movements.  Later, I was given CIPRO ear drops for an ear infection and had a very bad reaction.  my ear canal became incredibly itch and swollen, so I guess I have a sensitivity to it.

    My urologist mentioned that it might originate from my spine, but no tests were done.  I don't recall any injury or funny movements.  I don't really have any spinal pain.  My pain is all in my lower pelvic area.  I feel pressure against my rectum and bladder feeling a painful urge to urinate all the time.   Would welcome any suggestions.

    Hello Dr. Krongard,

     

    I had a TURP 16 days ago and overall it went really well.  My only concern, and it's a substantial one, is  regarding my orgasms.  I've had two orgasms since the TURP (on days 14 and 16 after the surgery) and the sensation I experienced during orgasm was a "2" whereas before surgery it was a "10."  My urologist said that it was highly unlikely that the orgasm sensation would change as a result of the TURP and all the literature I've read says the same.  Is it maybe because I'm having orgasms too soon after surgery?   I'm in overall great health and just 51 years old so the thought of living the rest of my life without at least decent orgasms is very depressing.  Any info you can provide would be very much appreciated.  Thank you.

    There is a general tendency, and maybe it applies to you, too, to rush and prove "manhood" in all its facets after prostate surgery. This reflects anxiety in most cases. And anxiety in itself can negatively affect orgasm, as well as many other functions and pleasures. I'd be interested to hear how you do not 2 weeks after surgery but 6 months after surgery.

    Went back to my PC a week & a half ago, the only antibiotic i was on was levofloxacin for 10 days by my urologist so my PC put me on sulfameth after 2 days bad reaction (light headed, cold sweat, burning urine, in frequent urination) stopped, switched to cipro 500mg 2x a day for a month,  taking a few vitamins and supliment super beta prostate.Been on a low acid ic diet since before the new year,spring water, not much change,very down. See my PC in a week or so,dont know whats next, hope he does
